This indicator measures the expenditure on the health care function "long-term care (health and social)” (SHA HC.3 and HCR.1) by the health care financing scheme "voluntary health care payment" (SHA HF.2) and by the health care financing scheme "household out-of-pocket payment" (SHA HF.3) in percentage of the gross domestic product (GDP) following the System of Health Accounts (SHA; OECD et al. 2011). Expenditure refers to the payments related to the final consumption of all health care goods and services by the resident units. Long-term care (health) consists of a range of medical and personal care services that are consumed with the primary goal of alleviating pain and suffering and reducing or managing the deterioration in health status in patients with a degree of long-term dependency. The function long-term care (social) comprises the expenditure on lower-level social care services to assist with instrumental activities of daily living (such as home-help, meals on wheels, transport and day centres, etc.). As the health care components of long-term care are contained in HC.3, the complementary social components are included as a health care-related category that is further subdivided into in-kind and in-cash components. The category voluntary health care finance schemes comprise all domestic pre-paid health care financing schemes under which the access to health services is at the discretion of private actors (though this “discretion” can and often is influenced by government laws and regulations). Included are: voluntary health insurance, NPISH financing schemes, and Enterprise financing schemes. The health care financing scheme household out-of-pocket payment comprises a direct payment for services from the household primary income or savings (no third-party payer is involved): the payment is made by the user at the time of the use of services. Included are cost-sharing and informal payments (both in cash and kind). The indicator represents the share of long-term care (health and social) expenditure by voluntary health care payment and by household out-of-pocket payments in relation to national income.
